Almost 80% of adults will have back pain at least once in their life. Lower back pain patients have back pain or/and stiffness in the lower back, especially after lifting heavy objects or after sitting for a long time.
What is Sciatica -
The sciatic nerve is a large nerve in our spine that is made up of several smaller nerves. Sometimes there is a slipped disc that presses on the sciatic nerve in the spine. It can cause back pain coming down from the back to the buttocks, thigh, and into the calf called as Sciatica. It can be very troublesome for the patient.
Common causes of back pain-
More than 90% of patients with back pain have a mechanical reason for back pain like a muscle strain or a slipped disc. These patients have more pain on lifting weights or physical exertion and are relieved by rest.
While 10% of adults with back pain have back pain due to a major disease like Ankylosing Spondylitis.
A. Mechanical causes -
Muscle strain - Causes of pain associated with back strain include muscular injuries from poor posture. We see so many patients working in the IT field suffering from back pain in Noida. Patients with muscle strain have pain limited to a local area but sometimes it may be spread to other areas also like myofascial pain.
Herniated disk - Herniated part of the disk may release some chemicals producing pain in the back. These patients again have either poor muscle tone or poor posture.
Lumbar spondylosis - Osteoarthritis affects the lumbar spine in patients who are in their 40s and 50s. These patients may develop either the disks degeneration (wear and tear of the supporting cushion in the back) or facet joint arthropathy resulting in back pain.
B. Systemic disease-
Infections like tuberculosis of the spine may cause night pain in the back. Patients feel pain all the time with fever, weight loss etc.
Ankylosing spondylitis may also cause severe back and buttocks pain, especially in younger males. This is a very important cause of back pain in males in their 20s and 30s and patients complain of a very stiff back. Unlike mechanical causes, it causes more pain on rest.
Fibromyalgia - This is also a very common cause of back pain in females who are in 30-50 years of age with lot of painful muscles, tiredness etc. We see many patients of Fibromyalgia in Noida who usually are not diagnosed properly.
C. Miscellaneous
Pain from the pancreas or gall bladder can also cause back pain.
Osteoporosis - Older patients who complain of back pain must be treated immediately. So many back pain in older people are due to a spine fracture due to Osteoporosis. These days early diagnosis and treatment can save a lot of these patients from fall and back pain.
Cancer (rare).
Genitourinary causes sometimes.
What happens to most people with back pain -
Most of the patients with low back pain will improve over 2-8 weeks. But a lower back pain sufferer may have on-and-off back pain in almost 75% of patients over the next year. In a few patients, back pain may persist for 1 year or longer.
Management
Back pain and sciatica pain may be treated with conservative management, interventional techniques, or surgical treatment.
1. Hot and cold compress - Initially after lower back pain, patients can do some cold compress and apply skin gel. A warm compress can be done after a few days.
2. Conservative care – After a brief period of rest, patients may do a few exercises after consulting a doctor. One can do walking or back exercises as per his/her comfort with good results.
3. Medications - Over-the-counter drugs like paracetamol may give relief if back pain is of a mild degree. Muscle relaxants and anti-inflammatory medications along with some nerve medications. can also achieve substantial relief in most patients.
4. Weight loss and postural modifications must be a part of back pain management at all stages. Patients get long-term benefits by optimizing their weight and avoiding back muscle strain.
5. Injections - Transforaminal or caudal injections reduce the inflammation and swelling of spinal nerve roots and reduce pain and numbness. Those patients who don’t get good relief with medications get decent relief with these injections.
6. Ozone discectomy - Another effective way to decrease back and leg pain. Ozone is injected under X-ray guidance to help prolapsed discs reduce in size.
7. Surgery - Only if all other options fail to relieve back pain. With so much advancement in the treatment of back pain and sciatica, the need for surgery is in only a limited number of cases.
